Supportive Housing – Litchfield (SHL)

Supportive Housing – Litchfield (SHL) is a Next Steps branch providing supportive housing to individuals who suffer from mental health issues or substance abuse. The program, in collaboration with The Connection, Inc., provides supportive housing and case management services for 10 individuals with severe and prolonged mental health and/or chronic chemical dependency.

 

SHL is an intensive home-based program that includes weekly home visits, budgeting, transportation and coordination of services, while providing rental subsidies. A case manager works with each client to develop a plan to achieve the client’s goals and objectives. The case manager works to identify safe, decent and affordable housing and supports are offered to acquire them.

 

The program serves individuals 18 year or older who are homeless or at risk of becoming homeless. The individual’s income must be at or below the 50% median income range. Involvement in the program is based on referrals only. Referrals are made by the Northwest Screening Committee at Charlotte Hungerford Hospital Behavioral Health Clinic in Torrington. Our program services the Northwest Corner of the state, Torrington and Winsted.
